Review & sign
Filing a birth worksheet ends with a person, not the system. The Review & sign page is where an attendant previews the pre-filled draft packet and attests to it, and we never file on your behalf — no worksheet is final until a signature is recorded against the record.
Open it from Review & sign on a record’s detail drawer. The button is a write action, so the read-only Viewer role doesn’t see it; Provider and Admin do.
A record has to be locked first
Section titled “A record has to be locked first”Signing attests to content that can no longer change in place, so only a Locked or Amended record is signable. Open the page on a still-editable record (Draft, Enrolled, or Complete) and you get a notice instead of a signature form:
This record is not ready to sign — a filing artifact is signed only on a locked record, so the attested content cannot change in place.
An Edit this record action takes you back to complete and lock the course of care first. (See Records & amendments for the lifecycle.)
Choosing the artifact and previewing the packet
Section titled “Choosing the artifact and previewing the packet”Pick the state at the top under State filing artifact — Washington, Oregon, or Vermont. Below it, a Packet preview renders the same pre-filled field map the PDF uses, so you review exactly what you’re attesting to rather than round-tripping through a download. While the artifact is unsigned it carries a diagonal Draft watermark and a Draft — review before signing tag; a badge tracks state:
- Draft — not yet signed (version N) before you sign.
- Signed — final for version N once a signature is on file.
Blank rows in the preview are the legal-identity fields the de-identified record never holds — you complete those on the form itself. A Download draft worksheet (PDF) link gives you the same packet as a file. The Washington, Oregon, and Vermont packets differ; Filing artifacts by state lists what each one includes.
Signing
Section titled “Signing”Signable records show an attestation checkbox:
I have reviewed this pre-filled draft worksheet and attest that the information is accurate and complete to the best of my knowledge.
Tick it and choose Sign filing artifact. The signature is recorded against the record’s current version.
Signatures on file
Section titled “Signatures on file”Each signature is kept in an append-only Signatures on file history, showing the signer, the state, the version signed, and when. Because a signature binds to a version, an amendment invalidates it: correcting a signed record cuts a new version, and that version needs its own signature before its packet is final again.
